i) The Driving Licence in original duly attested/ authenticated by the Home Department (General Administration Branch) of the concerned state and its photocopy.

ii) Sworn Affidavit (in duplicate) in the format prescribed.

iii) A photocopy of the first five pages and the page bearing the resident visa of applicant’s passport and his license.
